While the Department of Transport has overall responsibility for driver licensing, your local authority is a licencing authority and issue driving licences. Application forms and any other forms you may be required to submit with your application are available from the Motor Taxation Office of your local authority. Or you can access the provisional application form at www.oasis.gov.ie (Form D.201).
You will require the following when applying for your provisional licence:
- Provisional licence application: A completed application form with the appropriate fee €15.

Birth Certificate/Passport: Your application must be accompanied by a birth certificate or passport unless:
- You hold a current driving licence or provisional licence
- You held a driving licence within the last ten years or a provisional licence within the last five years
- You hold a current valid driving licence issued by another Member State if the European Union or another 'recognized country'. - Eyesight Report: A first time application for a provisional licence must be accompanied by a satisfactory eye report. This report may be completed by a registered ophthalmic optician or by a registered medical practitioner. You must sign the eyesight report in the presence of the optician or doctor.
- Photographs: Your application must be accompanied by two identical passport-type photographs. These should be signed on the reverse side of both photographs.

Medical Report: Your application must be accompanied by a medical report if:
- You are applying for a provisional licence in any of the categories C1, C, D1, D, EC1, EC, ED1 or ED.
- You will be 70 years of age or more on the first day of the periods for which the licence is being granted.
- You suffer from any alcoholism of epilepsy.
- You are taking, on a regular basis, drugs or medicaments which would be likely to cause the driving of a vehicle by you to be a source of danger.
Once you completed the form and compile the above data please forward the provisional application form to the licencing authority (Motor Taxation Office) in the County/City in which you reside.
